Printable blood pressure log sheet
A blank sheet to write readings on by hand, set up the way you want it rather than the way a fixed PDF was drawn. Choose how many rows you need, pre-fill the dates, add a pulse or weight column, make it large print, or print it in Spanish.
Nothing here is saved or sent. The sheet is generated in your browser and printed from there. If you would rather keep readings on your device than on paper, use the log.

What to write down
Write down exactly what the monitor displays — both numbers, and the pulse if it shows one. Note the date and roughly the time, because readings taken at different times of day are not directly comparable with each other. The measurement guide collects the American Heart Association’s published instructions on taking a reading worth recording.
A single reading does not describe a person, which is the whole reason a log is worth keeping: a clinician reading a fortnight of numbers can see something that no one number shows. To see where an individual reading falls in the published table, use the calculator.
